Eligibility

• People who, without basic maintenance and support services would be at risk of premature or inappropriate long term residential care including:-
  • Older or frail people with moderate, severe or profound disabilities
  • Younger people with moderate, severe or profound disabilities
• Unpaid carers of the above people

People do not qualify based on age or disability alone, but on difficulties in daily living and the need for ongoing assistance and supervision.

Access

The Seniors Coordinator will visit you and conduct an assessment to ensure you are eligible for assistance.  If a person meets the eligibility criteria there is no guarantee of services. Services are provided on the basis of relative need.
